How would you describe the mission efforts of your church in your community?
My church members live a missional lifestyle. They see their circles of influence as mission fields for the gospel.
My church regularly ministers in my community through outreach events.
Very few people in my church talk about ministry outside of the walls of the church.
Ministry to the community or outreach is never mentioned in my church.
Ministry and mission in our community is not even thought of.
What impact would be felt in the community if your church closed tomorrow?
The community would suffer without the congregation’s activity in the community and the church would be missed.
The community would suffer without the regular ministries of the church. But the community would not notice too much.
After a short time, the community would not notice if we were to close. Aside from our advertising, I am not sure they notice we are here now.

How would the reputation of your church be described in your community?
The community thinks my church is great! We are known as the people who love our community.
The community generally likes my church. We are known as the established church in our community.
The community is neutral toward my church. We have meet needs in the community but not enough to matter.
The community does not see my church as relevant or needed in their lives.
The community does not notice my church or is skeptical of my church because of a past event.
How missionally minded are your people?
My church is involved in several missions’ efforts, locally and abroad. Our missions’ offerings are great, and we exceed special giving goals. If there is a place to go my people want to go.
My church has their regular mission trips. Our missions’ offerings are strong and special giving offerings are good.
My church has a faithful few who go on a mission trip. Missions’ offerings are okay. Special mission giving is barely given any attention.
My church doesn’t have a mission trip. Our mission offerings are usually held until we can cover other expenses. My people see missions as something we will get to if we can.
We can’t do missions.
How equipped are your people to share the gospel?
In my church, congregation members regularly lead people to Jesus. My church regularly has opportunities to get equipped to evangelize and the members go out to witness.
My church regularly has training opportunities for evangelism. There are a few times each year for the church to witness for Christ. Mostly the pastor/staff are leading people to Christ.
My church has the same evangelism training opportunities it always has had. The same people always attend. Most salvations come from church events.
My church does very little regarding evangelism. There are some evangelistic materials around the church from years past, but nothing is in use. If we have a salvation, it is from an evangelistic event.
My church does not see a need to share their faith and so they do not. It has been a while since we have seen a salvation in my church.
How inviting are your church members?
My church members invite their friends regularly and we have a regular influx of new people because of it.
My church has visitors, although it there are not as many as there could be if my people were serious about inviting others.
My church has a few visitors each month. They usually come from church marketing or events.
I have not seen a visitor in a while.
We do not have visitors.
How welcoming are your church members?
Our small groups are growing rapidly because our people are friendly. After visiting my church, guests usually become members.
Our small groups are still growing but not as rapidly as we should. After visiting my church, guests sometimes join.
After visiting my church people rarely join. Our small groups are the same people they have always been.
When visitors do come, they don’t return. Our small groups are not growing.
Our small groups are getting smaller.
How willing are the church members to volunteer?
My church members are involved in everything. They have a strong desire to do all they can to see people saved and build the Kingdom.
My church members are good with volunteering for church ministries. But they do not serve outside of the ministries of the church.
My church members are okay with volunteering but rarely get involved without prompting. A minority of the congregation serves the majority of events.
My church members do not serve unless it is their pet ministry.
My church does not have an opportunity for members to serve.
How ready is your church to change?
My church is ready to do whatever it takes to be healthy and see God move. My church will adjust as needed to see the church grow and God glorified.
My church is willing to change if it makes sense, and they are walked slowly through change.
My church is reluctant to change anything. The way they’ve always done it has worked and will work in the future.
My church will not change. They see no reason to change. They are committed to preserving the past.
Change is not a possibility at my church.
What does the children’s ministry at your church look like?
The children’s ministry at my church is thriving! The ministry seeks to reach kids outside of the church, and we regularly see children saved.
The children’s ministry is healthy. We have sound structures in place that keep us on track.
The children’s ministry is good. We have children regularly but rarely do we see new growth.
The children’s ministry is not very healthy or active.
We do not have a real need for a children’s ministry.
How effective is your student ministry?
The student ministry at my church is thriving! Our students are bold in their faith and the gospel is reaching their peers.
The student ministry is good. They meet regularly and we see a good amount of student involved in the life of the church.
The student ministry is okay. They have their meetings, but we do not see them in church.
The student ministry is struggling.
We do not have a need for a student ministry.
How are the finances in your church?
The budget of my church is fully funded, and we are ready to go as God leads. We are putting aside funds for future ministry needs.
My church members are faithful in their giving. We are actively seeking God’s direction for how the funds should be allocated.
My church members are faithful in their giving. Our church gives as needs arise. Budget is met but we do not have a real purpose beyond the budget.
My church members are faithful in their giving. My church uses its budget on the needs of the church and the members.
The finances at my church need some serious help.
How ready is your church for the future?
My church has a God sized vision for future. We are excited at the size of the vision and how God will work to accomplish His vision through our church.
My church has a direction set for the next 3-5 years. We will keep going until we can do that.
My church is committed to the ministries it has always done. We will try something new, but we do not know what that looks like.
My church is committed to preserving the past.
I am not sure how much of a future my church has.
What does the preaching ministry of your church look like?
The preaching at my church is awesome! Engaging, Bible-driven, applicable messages are given in every service.
The preaching at my church is solid. Engaging, Bible-driven, applicable messages are constant.
The preaching at my church is consistent. We preach the Bible and are faithful to that effort.
The preaching at my church is scattered. Our preacher does the best he can but sometimes it is difficult to follow.
The preaching at my church is not good.
What does the discipleship ministry of your church look like?
My church has a clear pathway for discipleship at many levels. We regularly see people called to ministry and step into leadership.
My church makes disciples through programs and small groups. We see the fruit of that on occasion.
My church is program driven. We want to make disciples intentionally but are not sure how.
Making disciples in not on our thoughts. We study the Bible together, but spiritual progress is not the goal.
We have other needs that are more immediate than making disciples.
What does the spiritual maturity of your church look like?
My church has several people that I would call emotionally mature and spiritually mature. We regularly see people maturing in their faith.

My church has a few people that are emotionally mature and spiritually mature. It would be nice to have more.
My church has one or two I would consider emotionally mature and spiritually mature.
Emotionally mature and spiritual maturity would be difficult to gauge in my church.
What is the spiritual temperature of your church?
There is a clear presence of the Holy Spirit in my church. My people are hungry for God to move.
There are sporadic indicators of God’s movement in my church. My people are excited when it occurs.
The movement of God has cooled in my church. We desire for God to move greatly in our church.
It has been a while sense God moved in my church. It would be nice if He would again but that is not our focus.
There is no feeling of God’s presence in my church.
